SECTION 1
GREENFIELD BASEBALL ASSOCIAION (GBA) GENERAL RELEASE WAIVER
I hereby acknowledge that I am the legal guardian of the child/player listed above and that said child/player is voluntarily participating in sports activities organized by the GBA. I understand that these activities involve inherent risks, including but not limited to bodily injury, property damage, and death, which may occur as a result of my participation. Therefore, I agree to the following terms:

RELEASE AND WAIVER OF LIABILITY: I hereby release and forever discharge the GBA organization, its directors, officers, agents, coaches, employees, and volunteers (collectively, the "Released Parties"), from any and all liability, claims, demands, and causes of action that I may have now or in the future, arising out of or related to any loss, damage, or injury, including death, that may be sustained by my child or to any property belonging to my child, whether caused by the negligence of the Released Parties or otherwise.

ASSUMPTION OF RISK: I understand and acknowledge that my participation in the sports activities organized by the GBA organization involves inherent risks, including but not limited to bodily injury, property damage, and death. I voluntarily assume all risks associated with my child's participation in these activities.

INDEMNIFICATION: I agree to indemnify, defend, and hold harmless the Released Parties from any and all claims, demands, and causes of action, including attorneys' fees and costs, arising out of or related to my child's participation in the sports activities organized by the GBA organization.

MEDICAL TREATMENT: I authorize the GBA organization, its agents, employees, coaches, and volunteers, to obtain or provide medical treatment for my child in the event of any injury, illness, or other medical condition that may occur during my child's participation in the sports activities.

ACKNOWLEDGEMENT OF UNDERSTANDING: I have read this sports waiver, fully understand its terms, and understand that I am giving up substantial rights, including my right to sue the Released Parties for any injury, death, or damages that may result from my participation in the sports activities organized by the GBA organization.

SECTION 2
GREENFIELD BASEBALL ASSOCIATION (GBA)
WAIVER/RELEASE FOR COMMUNICABLE DISEASES INCLUDING COVID-19
ASSUMPTION OF RISK / WAIVER OF LIABILITY / INDEMNIFICATION AGREEMENT
In consideration of being allowed to participate on behalf of GREENFIELD BASEBALL ASSOCIATION (GBA) athletic program and related events and activities, the undersigned acknowledges, appreciates, and agrees that:

Participation includes possible exposure to and illness from infectious diseases including but not limited to MRSA, influenza, and COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist; and,

I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my child's participation; and,

I willingly agree to comply with the stated and customary terms and conditions for participation as regards protection against infectious diseases. If, however, I observe and any unusual or significant hazard during my child's presence or participation, I will remove my child from participation and bring such to the attention of the nearest official immediately; and,

I, for myself and on behalf of my child, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS GBA their officers, officials, agents, coaches, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (“RELEASEES”), WITH RESPECT TO ANY AND ALL ILLNESS, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF RELEASEES OR OTHERWISE, to the fullest extent permitted by law.
FOR PARTICIPANTS OF MINORITY AGE (UNDER AGE 18 AT THE TIME OF REGISTRATION)
This is to certify that I, as parent/guardian, with legal responsibility for this participant, have read and explained the provisions in this waiver/release to my child/ward including the risks of presence and participation and his/her personal responsibilities for adhering to the rules and regulations for protection against communicable diseases. Furthermore, my child/ward understands and accepts these risks and responsibilities. I for myself, my spouse, and child/ward do consent and agree to his/her release provided above for all the Releasees and myself, my spouse, and child/ward do release and agree to indemnify and hold harmless the Releasees for any and all liabilities incident to my minor child’s/ward’s presence or participation in these activities as provided above, EVEN IF ARISING FROM THEIR NEGLIGENCE, to the fullest extent provided by law.

SECTION 3
GREENFIELD BASEBALL ASSOCIATION (GBA) SERVICE DUTY COMPACT OF UNDERSTANDING
I understand that as a volunteer-run organization, the GBA relies on fundraising and community service duties to function and that my participation in various activities as a representative of my child is necessary. I understand that such service duties may include, but are not limited to, fundraising, pop-stand coverage, and special event support. I understand that the GBA leaders and Board of Directors are volunteers who will make these opportunities known as needs arise. I commit to doing my part to support the growth of GBA so that all children may be provided the opportunity to grow their experiences with baseball.
SECTION 4
GREENFIELD BASEBALL ASSOCIATION (GBA) PHOTO RELEASE
As legal guardian of the above listed child/player, I understand that GBA will take photos and/or videos in any manner to help promote the league activities as determined in the sole discretion of the Board of Directors. Such use could include publications, media releases, public announcements, electronic or otherwise, and on league websites or social media pages. I agree that neither I, nor the above listed minor, will receive any compensation if such image appears in any of the manners listed above or other manner that the league deems appropriate. I agree that such image is the property of GBA.

GBA also understands that some parents do not want photos of their child to be taken. We respect that decision and will make a GOOD FAITH EFFORT to NOT post photos of your child if you prefer. Parents who do NOT want photos of their child posted MUST email gfbaseball1951@gmail.com explaining this and we will do our very best to honor this request.
